Applying for the program itself
Communicare Elite cannot approve you for Consumer Directed Services — only the Missouri Department of Health and Senior Services can. To request an assessment, contact the DHSS Home and Community Based Services line at 866-835-3505. We can walk you through it first if you would like. See the step-by-step process.
