What a personal care attendant actually does
This page is plain on purpose. People quit this job because it was not what they pictured. An honest description up front prevents that.
The core of the job
You help one person do what they can no longer do alone. It happens in their home, on the schedule they set, and covers only the tasks the state approved.
Personal care
- Bathing and showering
- Shampooing and hair care
- Dressing and undressing
- Shaving and grooming
- Oral hygiene and denture care
- Nail care
- Toileting and incontinence care
Mobility
- Transfers — bed to chair, chair to wheelchair
- Repositioning to prevent pressure sores
- Walking support and stability
- Help with canes, walkers, lifts

- Meal preparation and help with eating
- Laundry and bed linens
- Light housekeeping in the areas used
- Grocery shopping and errands
Transportation
- Medical appointments
- Grocery shopping and essential errands
- School or employment
Essential trips only — this is not a general driving service.
What the job is not
- It is not nursing. No injections, no sterile wound care, no IVs, no tube feeding management, no clinical assessment.
- It is not therapy. Physical, occupational and speech therapy are separate, licensed services.
- It is not housekeeping for the household. You support the consumer, not their relatives or roommates.
- It is not decision-making. The consumer directs their own care. Your job is to do what they have asked, the way they have asked.
- It is not open-ended. Only tasks in the authorized care plan are billable.
The parts people find hard
Said plainly, so nothing catches you off guard:
- Intimate care. Toileting, incontinence and bathing are a routine part of the work. It is uncomfortable at first for almost everyone.
- Physical strain. Transfers and support work are demanding. Learn safe technique and refuse anything genuinely unsafe.
- Emotional weight. You may be the only person your consumer sees some days. That is meaningful and it is also heavy.
- Boundaries. Consumers may ask for things outside the plan. Saying no kindly, and consistently, is a real skill.
- Family dynamics. Especially when the consumer is your own relative and you are now also their employee.
The parts people find worth it
The person you help stays in their own home instead of a nursing home. That is the whole point of the program, and it happens because somebody shows up.
The attendants who last are the ones who find that genuinely satisfying. Not the ones who took the job because it was going.
