Where Communicare Elite provides Consumer Directed Services
We serve St. Louis City, St. Louis County, and Missouri communities within roughly 40 miles of our Woodson Road office. That radius covers most of the Missouri side of the metropolitan area, including all of St. Charles County, the populated half of Jefferson County, and eastern Franklin and Lincoln counties.
Missouri only. Consumer Directed Services is a Missouri Medicaid program administered by Missouri agencies. We do not serve Illinois residents, even those a few minutes across the river — Illinois runs its own separate program through its own providers.
Communities we serve
Every community below has its own page with local detail. If you do not see your town, call (314) 809-6655 — if you are in Missouri and within about 40 miles of us, we can almost certainly help.

Does my address qualify?
Two things decide it, and neither depends on your zip code alone:
- Are you eligible for the program? You must be a MO HealthNet participant, 18 or older, have a physical disability, be able to direct your own care, and be assessed at a nursing facility level of care. Full eligibility rules.
- Can we reach you? Our attendants and staff need to be able to serve your home. Within about 40 miles of St. Louis, in Missouri, the answer is nearly always yes.
Why the 40-mile limit exists
It is not an arbitrary line drawn around an office. Consumer Directed Services depends on someone physically arriving at a home, often every day, frequently early in the morning. A service area drawn wider than the staff and attendants can reliably cover produces exactly one outcome: people who were promised help and do not get it on the days it snows.
The practical consequence is worth planning around. Because you hire your attendant rather than waiting for an agency to staff your case, the distance that matters most is not between you and us — it is between you and the person you want to employ. An attendant who lives fifteen minutes away is materially more reliable over a year than one who lives an hour away, however well-intentioned.

Why we do not cross into Illinois
People are often surprised by this, because the Illinois side of the metro is closer to our office than Wentzville is. The reason is not geography — it is that Consumer Directed Services is a Missouri Medicaid program. Eligibility is determined by Missouri agencies, the care plan is authorized by the Missouri Department of Health and Senior Services, and the services are paid for by MO HealthNet.
An Illinois resident is not eligible for MO HealthNet, so there is no Missouri program for a Missouri vendor to administer on their behalf. Illinois operates its own consumer-directed programs through its own providers. If you live in Illinois, that is where to start — and we will say so plainly rather than taking your details and wasting your time.
