This policy covers information collected through communicareelite.com and the ways we communicate with you about it. It is separate from the HIPAA Notice of Privacy Practices below, which governs your protected health information as a patient.
Information we collect
- Information you give us — name, contact details, and anything you enter into a form or send us by email or text.
- Technical information — IP address, browser and device type, pages viewed and time on site, collected automatically through cookies and similar technologies.
How we use it
- To respond to you, schedule appointments, and provide the services you asked for
- To send the communications described in our SMS Policy, where you have consented
- To operate, secure and improve the website
- To meet legal and regulatory obligations
What we do not do
We do not sell your personal information. We do not share your mobile number with third parties or affiliates for their own marketing. We share information with service providers who work on our behalf — and only as far as they need it to do that work.
Cookies
You can refuse or delete cookies through your browser settings. Parts of the site may not work properly if you do.
Security
We use administrative, technical and physical safeguards to protect the information we hold. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security. Please do not send health information through this website’s forms, by email, or by text.
Children
This website is not directed at children under 13, and we do not knowingly collect their information online.
Your choices
You may ask us what information we hold about you, ask us to correct or delete it, and opt out of messages at any time. Contact us using the details at the end of this page.
HIPAA Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Communicare Elite is required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your health information
- Treatment — to provide, coordinate and manage your care, including sharing information with other clinicians involved in treating you.
- Payment — to bill and collect payment from you, your insurer or another payer.
- Health care operations — for quality assessment, staff review, training, licensing and business management.
- Business associates — vendors who perform services for us, such as billing, transcription or IT support. Each is bound by written contract to safeguard your information to the same standard we are.
- As required by law — including public health reporting, reporting abuse or neglect, health oversight activities, judicial and law enforcement requests, and to avert a serious threat to health or safety.
- Organ and tissue donation — to organizations that handle procurement or transplantation, where relevant.
- Workers’ compensation — as authorized by, and to the extent necessary to comply with, workers’ compensation law.
- Military, veterans and national security — where required by military command authorities or for authorized national security activities.
- Correctional institutions — if you are an inmate, to the institution or a law enforcement official where necessary for your health or the safety of others.
- Research — where an institutional review board has approved the study and put privacy protections in place.
Other uses and disclosures require your written authorization, which you may revoke at any time in writing. This includes most uses for marketing, any sale of your information, and most uses of psychotherapy notes. If we contact you for fundraising, you may tell us to stop.
Your rights
- Inspect and copy your health record, including an electronic copy.
- Request an amendment if you believe your record is incorrect or incomplete.
- Receive an accounting of certain disclosures we have made.
- Request restrictions on how we use or disclose your information. We are not always required to agree — but if you pay in full out of pocket for a service, you may restrict disclosure of that service to your health plan and we must honor it.
- Request confidential communications at an alternative address or by an alternative means.
- Receive a paper copy of this notice, even if you agreed to receive it electronically.
- Be notified if a breach occurs involving your unsecured PHI.

We may change this notice and make the new terms effective for all information we hold. The current notice is always posted here with its effective date.
